Examination and RTO requirements

Form 1A requires a practitioner appointed or authorized for this purpose by the state government. The form records a personal medical examination, including vision and hearing checks. Telemedicine guidance does not waive these requirements. An in-person or specialist assessment may be needed before a certificate can be issued. Read the official Form 1A requirements.

Every Form 1A carries the issuing doctor's name, State Medical Council or National Medical Council registration number, signature and a unique verification ID with a QR code, alongside direct upload to Parivahan, so your RTO can confirm exactly who certified your fitness and when.

What is Form 1A medical certificate?
Form 1A is a medical fitness certificate required by the RTO for driving licence renewal (age 40+) and heavy motor/transport vehicle licences. It certifies that the applicant is medically fit to hold a driving licence.
Who needs Form 1A in India?
Form 1A is required across all states for: (1) anyone renewing a driving licence aged 40 and above, (2) heavy motor vehicle licence holders, and (3) transport/commercial licence applicants. Check your Parivahan application for the specific requirement shown for your state.
Can Form 1A be done online?
